What is Long-Term Care at Prestige?

Long-term care, also known as extended care, is intended to help residents who require continuous assistance with activities of daily living in addition to skilled nursing care. This level of care provides patients with 24-hour care designed to support individual medical needs and may include a combination of a customized diet, restorative therapy, and assistance with daily activities.

When is it Time for Long Term Care?

Long-term care is recommended when a resident’s care needs are no longer able to be met at a lower level of care. We provide this specialized care when your loved one needs assistance with activities of daily living, such as bathing, dressing, and eating and/or when your clinical care needs require 24/7 nursing oversite.

What Can You Expect from a Long-Term Care Center?

A long-term care center, often referred to as a nursing home, provides patients with 24-hour care designed to support their ongoing clinical and rehabilitative needs. Our interdisciplinary team will work with you to build an individualized care plan tailored to your preferences and needs. In addition to 24/7 oversite, we use a holistic approach focused on enhancing your well-being through activity programming, social services support, dietary support, and more.

Data-Driven Approach to Recovery

Our experienced therapy team uses a data-driven analytical approach to increase positive outcomes utilizing proven measures like Berg Balance Scale (BBS), 2 Minute Walk Test (2MWT), and Functional Reach Test (FRT) and others to dramatically decrease fall risk or rehospitalization and improve overall functional strength. Each person’s individual therapy treatment plan consists of physical therapy, occupational therapy and/or speech therapy.

Occupational Therapy: focuses on achieving independence in activities of daily living, refining motor skills, improving hand and upper extremity function, and bowel/bladder control. Speech Therapy: focuses on improving communication in speaking, problem-solving, reading, writing, and verbal expression along with resolving problems with swallowing. Physical Therapy: focuses on improving functional skills, strength, and mobility.

Discharge and Transition

At Prestige, our goal is to help you transition from hospital to home and provide you with tools for each phase of your recovery. From discharge at the hospital to transition to one of our skilled nursing centers, our dedicated team is here for you every step of the way.
