If you or your loved one have been hospitalized following a heart attack or have recently undergone heart surgery, your doctor may suggest cardiac rehabilitation at a Skilled Nursing Facility (SNF) to continue recovery and ease the transition from hospital to home. Cardiac trauma can be unexpected and may lead to life-long chronic medical conditions or rehospitalization, if left untreated. However, skilled nursing rehabilitation after a cardiac event ensures a patient is able to recover in a safe and encouraging environment and can return to their active and independent lifestyle.

At a Prestige Care Skilled Nursing and Post-Acute center, our interdisciplinary team partners with each patient and their family to build an Individualized Care Plan to support their recovery goals and prevent rehospitalization. We discuss cardiac care options and conduct an evaluation to develop a rehabilitation plan tailored to every patient’s unique medical needs. Our clinical team is dedicated to helping each cardiac patient manage their heart disease, provide them with education on maintaining a healthy lifestyle, and support their recovery goals with a personalized therapy plan. In addition to continuous support and education, patients receive 24-hour care focused on enhancing their well-being through restorative exercise to improve cardiac fitness, assistance with daily activities, and social services support.

Our comprehensive cardiac program provides each patient with a phased approach to recovery with the goal of becoming as independent as possible. The first phase of recovery may involve education on what to expect during your rehabilitation, working with your clinical team to build a recovery plan, developing a heart healthy diet with your dietary team, and learning how to improve mobility. During the second phase of recovery, patients may find greater ease in accomplishing daily tasks, increasing exercise, and learning how to maintain a healthy lifestyle following cardiac recovery. The third, and final phase of recovery provides patients with the tools necessary to support a successful discharge from our skilled nursing center. During this phase, patients will receive education on how to continue recovery at home and a final evaluation to determine the need for home health or outpatient services.

Each person’s cardiac rehabilitation journey is unique to their own medical needs and may include a variety of rehabilitation approaches such as education, exercise therapy and personalized meal plans for a heart healthy diet. Maintaining a Healthy Heart

To prevent rehospitalization, it’s important to understand how to manage your heart disease. At Prestige, your clinical team will work with you to create a personalized discharge plan for your return home. The CDC also provides helpful tips for those recovering from cardiac trauma and those who are at a higher risk for heart disease:

  • Understand your heart health history and your risk for heart disease.
  • Eat a heart healthy diet of fruits, vegetables, whole grains, and protein-rich food.
  • Exercise daily.  
  • Monitor your blood pressure.
  • Reduce alcohol and tobacco consumption.
